Awful. The guide was impolite and rushed through a 3-hour tour in 2 hours. Some rooms were dangerously overcrowded. Overall, it was a very disappointing experience.

Access to St. Peter's Basilica was denied despite our booking and payment, so no tour there. We only visited the Vatican Museum and Sistine Chapel. Even the tour guide was surprised. This means paying again and queuing if you want to see the Dome.
